During
your initial appointment Dr. Martone
will perform a thorough examination prior to any treatment. The examination
will include a thorough review of your medical and dental history, taking
and reading any necessary xays, and a complete oral exam. From the examination
Dr. Martone
can determine your diagnosis and recommend treatment options.
Minor treatment plans
and procedures can often be preformed the same day as the exam and consultation,
but for more complex treatment plans or medical history concerns, we will
set up a second appointment for your treatment.
Information
that can assist us at your initial appointment
- Please bring your
referral slip from your doctor and any current x-rays they have taken.
- Please bring a list
of your medications that you are currently taking including aspirin and
vitamins.
- Be prepared to alert
us of any health condition (for example: high blood pressure, diabetes,
heart conditions, artificial joints or valves, or a past history of rheumatic
fever)
- Bring your medical
and dental insurance information. If your insurance is through a family
member, please bring their information as well.
- Patients under the
age of 18 years must be accompanied by a parent or legal guardian.
